Mindset – Foundations

Foundations is a medium to long-term psychosocial outreach service that provides evidence-based, goal-oriented capacity building support to people receiving clinical support from Mental Health Services.

Duration: 18 months max

Who this service is for

Foundations accepts referrals from mental health case managers within DHHS only.

Mental Health Case Managers can refer through their Clinical Lead or email foundations@baptcare.com.au

Age: 18-64 years

The Horizons team provides short-term, outreach support assisting people in building strategies, skills and resources to work towards individual goals for their personal recovery. Individual, client-led support can be provided for up to 26 weeks.
