Our Support Coordinators here at Just Better Care Hobart have a wide range of experience within the NDIS and the wider community. Our role is to assist you to understand your NDIS Plan and how it can be utilised to support you, as an NDIS participant, in the most effective and efficient way possible. This can include any of the following:

– Connecting with you to develop your knowledge of your NDIS plan so you can better manage the NDIS into the future.
– Connection with Service Providers.
– Connection with Allied Health Providers including Occupational Therapy, Physiotherapy, Speech Therapy, Exercise Physiology etc.
– Connection with health providers including nursing and can include working alongside your GP, Medical Centres etc.
– Accessing reports and assessments to ensure ongoing NDIS support and increased funding where required.
– Arranging equipment such as hoists, continence products and other assistive technology.
– Working with housing providers and builders to develop ‘Bricks and Mortar’ solutions or SDA installations.

Basically, our Support Coordinators are here to work alongside you on your NDIS journey and utilise their knowledge and experience to guide you and inform you of the best paths along that journey.

How this service is delivered

• Face-to-face

  Support Coordination services can be provided face to face, over the phone, online or across all of these mediums of communication. We can meet you in your own home, or you are welcome to come into our Hobart office.
