An initial consultation generally involves meeting in person with you and a family member or friend of your choosing to openly discuss your care needs and wishes, usually in your home.
The consultation reflects a holistic approach, considering your physical, mental, and social wellbeing. Drawing on a range of evidence-based principles and frameworks to understand your whole context, ensuring the support offered is person-led, tailored to what matters most to you.
