HR+ is a Tasmanian NDIS registered service provider that specialises in support coordination and plan management for participants living with a disability. Our plan managers and support coordinators are a committed and respectful team that offer a personalised service, face-to-face visits and who utilise the knowledge from the broader HR+ team, not just from a single team member.
Our team can help you to build the skills you need to understand your plan, connect you to organisations that provide supports and work with you to make sure that you have the right mix of these supports to make the most out of your plan, live more independently and feel included in your community. We can also help you plan ahead to be ready for your plan reviews.
