Care Finder program
The Care Finder program provides intensive, personalised support to older Tasmanians who need help accessing aged care and community services.
Care Finders work alongside people who may be disconnected from the aged care system, unsure how to use My Aged Care, or facing barriers such as limited support networks, homelessness, or complex life circumstances. The program is designed for people who need more than basic information and benefit from hands-on guidance.
Care Finders help individuals to:
- Understand and interact with My Aged Care
- Navigate the aged care system and explain assessment processes
- Access aged care services and relevant community supports
- Find useful resources within their local community
- Stay connected through regular check-ins to ensure ongoing needs are met
The service supports older people who are not yet receiving aged care services, as well as those already in the system who require additional help. It is particularly suited to people who do not have family, friends, carers, or representatives to assist them, or who prefer support from an independent, trusted service.
The Care Finder program in Tasmania is delivered by The Salvation Army and funded through Primary Health Tasmania under the Australian Government’s Primary Health Networks Program.
Launceston Care Finder Service
The Care Finder program is also available at The Salvation Army Corps Church Launceston. This multi-use hub welcomes clients to drop in, participate in the breakfast program, and easily access support by speaking with the Care Finder team.
Hoarding and Squalor Program
Funded through the Commonwealth Home Support Program (CHSP), this specialized service supports older, low-income individuals experiencing hoarding behavior or living in squalid conditions. It aims to prevent homelessness and help clients access the aged care services they need.
The program provides one-off forensic cleaning by professional contractors to prevent eviction and enable the commencement of aged care support. To access this service, clients must have an Aged Care Client ID and a valid My Aged Care (MAC) referral code specific to Hoarding and Squalor.
Referrals can be made by assessors through the MAC Portal, while other service providers can refer clients directly via email at tasmaniacarefinders@salvationarmy.org.au.
Who this service is for
- People aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islander people)
- Prematurely aged people over 50 who are homeless or at risk of homelessness
- People who need help with one or more everyday tasks
- Referrals can be made by anyone, including individuals, carers, family members, or service providers.
